Regents Actions in 51 Professional Discipline Cases and 3 Restoration Petitions

The Board of Regents announced disciplinary actions resulting in the revocation of 2 licenses, surrender of 17 licenses, and 32 other disciplinary actions. The penalty indicated for each case relates solely to the misconduct set forth in that particular case. In addition, the Board acted upon 3 restoration petitions.

I. REVOCATIONS AND SURRENDERS

III. RESTORATIONS

The Board of Regents voted on October 17, 2023 to deny the application for restoration of the physician license of Pavel I. Kulik, Brooklyn, NY.  Dr. Kulik’s license was revoked on March 16, 2015.

The Board of Regents voted on October 17, 2023 to stay the execution of the order of surrender of the registered professional nurse license of Matthew D. Mendelow, New York, NY, to place him on probation for one year under specified terms and conditions, and upon successful completion of probation, to fully restore his license.  Mr. Mendelow’s license was surrendered on February 23, 2016.

The Board of Regents voted on October 17, 2023 to deny the application for restoration of the licensed practical nursing license of Suzette Guzman-Moore, Long Island City, NY.  Ms. Guzman-Moore’s license was surrendered on September 16, 2014.
